Likely Buccal Bifurcation Cysts on Both a Maxillary and a Mandibular Molar

The Finding

An 8-year-old female was referred after the practice noted a radiolucency buccal to tooth #19 and observed that tooth #14 also appeared to be pushed buccally, without an apparent reason. The radiologist identified low soft tissue density lesions on the buccal aspects of #19 and #14, slightly tipping the roots of #14 palatally and #19 lingually. These were suggestive of inflammatory cysts, likely buccal bifurcation cysts.

The radiologist noted that buccal bifurcation cysts have been documented with mandibular first molars, but multiple lesions are rare, particularly involving a maxillary molar, making this a very rare presentation of two buccal bifurcation cysts, one in the maxilla and one in the mandible.
