An 11-year-old male with no significant medical history was referred for a general read with a focus on the right maxillary sinus. The radiologist identified a large lesion associated with a grossly displaced tooth #4 that occluded the entirety of the right maxillary sinus airspace, with expansion of the sinus borders and occlusion of the right maxillary ostium. The radiographic features were most suggestive of a benign odontogenic cyst or tumor, and biopsy was recommended for definitive diagnosis. The radiologist noted that the size of the lesion indicates it had been growing for a significant period of time, likely without symptoms, and that prompt treatment is needed to avoid further sinus and dental complications.
